INSURANCE & PAYMENTS

Dawes Family Medicine is pleased to accept most major PPO insurance and Medicare. 
 
Please remember that insurance coverage is a contract that exists between you and your insurer.  Patients are responsible for co-pays, co-insurance,and deductibles under their specific insurance policy.  It is the policy of Dawes Family Medicine to collect the annual membership fee,  co-pays, patient balances and deductible amounts at the time of service. 
 
We encourage every patient to contact their insurance company to verify if we are in your network and find out what is covered.  Dawes Family Medicine makes every effort to verify patient eligibility, however, as contracted providers the determination of patient financial responsibility will ultimately be made by the insurer.
 
As a courtesy, we will bill your primary insurance company for you.  
The patient is responsible for paying any balance not covered by insurance.

Additional Fees Not Covered by Insurance:
 

2023 ANNUAL OFFICE FEE (Per Calendar Year) $150 per Individual, $100 for students aged 21-26, $300 max per family.

Covers billing and insurance claims, medications, electronic health records, patient portal and all administrative costs outside a patient visit. Not applicable to Physician's Choice Patients.

We are not a provider of Covered California.

 

We offer the self pay fee schedule for those who do not have any insurance or have insurance with a company that we are not contracted with.
